Membership Hold Form

  • By submitting this form, I am giving CrossFit Incendia my SEVEN (7) DAYS written notice to place my membership on hold per my signed membership agreement.

    I understand that I may place my account on hold only one (1) time per calendar year, for up to three (3) consecutive months.

    I understand that a $25 fee will be assessed per hold request.

    I understand that holds run from the 1st of the month to the 1st of the month and that holds for less than one (1) month are not available.

    I understand that holds cannot be applied retroactively.

    I understand that my draft will automatically restart after my hold period.

    I understand that I will not have access to CrossFit Incendia facilities or classes, Wodify or Facebook Groups during my hold period.

    I understand that if I cancel during the Hold Period, I must submit a separate Cancellation Form and it will be subject to the Cancellation Policy per my Membership Agreement.

    I also understand that NO REFUNDS of membership dues will be given if I fail to make my billing change request less than SEVEN (7) DAYS prior to the 1st of the month.

Membership Cancellation Form

  • By submitting this form, I am giving CrossFit Incendia my SEVEN (7) DAYS written notice to cancel my membership per my signed membership agreement.

    Should I choose to rejoin CrossFit Incendia after the termination of my membership, I understand that I will have to re-join at the current membership rate and my current rate may not be preserved.

    I understand that membership cancellations must be requested IN WRITING at least SEVEN (7) DAYS prior to the 1st of the month I wish to make my billing change and that if this request is outside of that time period, my account will be charged in full for that month.

    I agree that my membership cancellation will occur the following month if less than SEVEN (7) DAYS notice is given.

    I agree that Membership Cancellation requests made by any other medium will not be honored (i.e. verbal requests, Facebook messages, notes left on the front desk, phone calls, etc).

    I understand that I will not have access to CrossFit Incendia facilities or classes, Wodify or Facebook Groups once I cancel my membership.

    I also understand that NO REFUNDS of membership dues will be given if I fail to make my billing change request less than SEVEN (7) DAYS prior to the 1st of the month.
